Friend moved and gave me this. Pretty heavy, couple pounds. Does it go from AC to AC or DC? What would it be used for?

I would say that s prob a transformer for low voltage landscape lights.

Thanks Brett. I have found a few Jefferson Electric 216-1121 transformers online but they don't say if it's AC to AC or DC. Not sure what I would use it for anyway. Now if it went 12vdc to 110ac like an inverter, that I could use.

Usually a transformer is just stepping voltage up or down not changing it from A.C. to DC, a rectifier does that.

I suspect this is just a 120 volt ac to 12 volt ac transformer. Besides outdoor lighting, not sure what else to do with 12 volts ac?

Transformers change AC voltages up or down.
There are devices that change DC voltages up or down They are called 'buck converters' if they change voltage down and are called 'boost converters' if they change the voltage upwards.

12 & 24 volts are used for control systems in machines.
Buck Boosts is a generic term and can be AC or DC
